Please be advised that QWEST Facilities sustained damage as a result of the negligent acts by
employees or agents of the CITY OF BOZEMAN, MT.
Investigation has revealed that on or about 6/29/2007, employees or agents ofthe CITY OF
BOZEMAN, MT damaged A BURIED CABLE while doing WATER EXCAVATION WITH A
BACKHOE in area at 19TH AND BAXTER LN, BOZEMAN, MT.
If there is insurance to cover this matter, kindly advise as to the name of the insurance
company, its address and the claim number assigned.
