Please be advised that QWEST Facilities sustained damage as a result of the negligent acts by
employees or agents of the CITY OF BOZEMAN, MT.SECURITEC SAFETY SYSTMES INC.
Investigation has revealed that on or about 5/7/2007, employees or agents of the CITY OF
BOZEMAN, MT SECURITEC SAFETY SYSTMES INC. damaged BURIED CABLE while
doing HIGHWAY/STREET EXCAVATION WITH A BACKHOE in area at 34 WILDCAT,
BOZEMAN, MT.
If there is insurance to cover this matter, kindly advise as to the name of the insurance
company, its address and the claim number assigned.
