Please be advised that QWEST Facilities sustained damage as a result of the negligent acts by
employees or agents ofthe CITY OF BOZEMAN, MT.
Investigation has revealed that on or about 4/5/2007, employees or agents of the CITY OF
BOZEMAN, MT damaged BURIED CABLE while doing CULVERT EXCAVATION WITH A
BACKHOE in area at COTTONWOOD, BOZEMAN, MT.
If there is insurance to cover this matter, kindly advise as to the name of the insurance
company, its address and the claim number assigned.
REQUEST FOR GOVERNMENTAL NOTICE FORM
