General Permit for East Lake Professional Center,MTG071090
Dear Mr. Horner:
H&H Earthworks is authorized by the Montana Department of Environmental Quality(DEQ)to discharge
under the Construction Dewatering General Permit(CDGP)for dewatering at the discharge location listed
below.

Please read the General Permit to ensure that you understand how to comply with the conditions including:
• Keeping a copy of this authorization letter and the 2020-issued CDGP on-site at all times;
• Implementing a site-specific Dewatering Control Plan, including taking corrective actions if needed;
• Conducting self-monitoring with the frequency associated with the Dewatering Category listed above
and completing a daily log to be maintained on-site; and
• Submitting Discharge Monitoring Reports(DMRs)electronically via NetDMR at
https:Hnetdmr.zendesk.com/hc/en-us.You are required to submit monthly DMRs for the duration of your
authorization, even if you do not discharge. DMRs are due by the 281 of the following month.If you
have questions about submitting DMRs, please call(406)444-0574.
Please be aware that annual fees continue to accrue and are invoiced in arrears until the authorization
is terminated or the CDGP expires on February 28,2025. Please find a blank notice of termination(NOT)
form enclosed. The NOT should be submitted only after dewatering is complete.
