Construction Occurring in Phase:  Strip weeds;  Clear and grub top 9-inches of entire area of work; Site Best Management Practices:  Install gravel construction entrance pad per City Standards;  Store equipment and stockpile material in designated area;  Perform street sweeping after vehicle tracked sediment occurs on public streets;  Cover stockpiled material;  Perform any equipment maintenance or re-fueling in a protected lined area;  Immediately repair any leaks on equipment;  Have a watering truck available for wind blowing dust;  Provide a trash receptacle with scheduled pickup;  Install sanitary facility for workers, stake down (not near top of slope);  Earthwork Contractor shall perform biweekly inspections;  Maintain spill kit on site; Phase II: Utility Installation Potential Pollutants:  Sediment laden stormwater;  Equipment petroleums from re-fueling, lubrication, leaks;  Vehicle tracking soils onto public streets from the site due to tires catching disturbed soils on-site;  Human generated garbage;  Human sanitary waste. 2 Construction Occurring in Phase:  Installation of sanitary sewer main and services and related appurtenances;  Installation of water main and services and related appurtenances;  Installation of storm drain system and related appurtenances;  Installation of dry utilities conduit;  Sanitary Sewer and Water Main testing. Site Best Management Practices:  Install erosion control measures and BMP's in locations as noted on Phase II Site Plan;  Maintain gravel construction entrance pad;  Store equipment and stockpile material in designated area;  Store chemicals, petroleums, gasoline, fuel and other containered items in a locked trailer or shed.  Maintain sanitary facility for worker use;  Maintain trash receptacle for human generated trash;  Flush chlorinated water mains into an excavated hole and allow to infiltrate.  Sweep streets immediately to remove any sediment tracked from trucks exiting the site;  Utility Installer shall perform biweekly inspections;  Maintain spill kit on site;  Groundwater point wells to be installed for dewatering purposes. Clean water from the dewatering process to be piped to the nearest irrigation ditch. Phase III: Asphalt, Curbing, and Sidewalk Potential Pollutants:  Sediment laden stormwater;  Equipment petroleums from re-fueling, lubrication, leaks;  Vehicle tracking soils onto public streets from the site due to tires catching disturbed soils on-site;  Nuclear soil density gauge being struck;  Human generated garbage;  Human sanitary waste.  Concrete waste generated from trucks and testing; Construction Occurring in Phase:  Rough grading of entire site;  Excavation for placement of road subbase material  Installation of pitrun gravels;  Installation of roadmix gravel cushion;  Watering of gravels;  Pouring concrete for sidewalks;  Pouring concrete curb & gutter; 3  Placement of Asphalt Pavement;  Nuclear gauge testing of soils. Site Best Management Practices:  Install erosion control measures and BMP's in locations as noted on Phase III Site Plan.  Store equipment and stockpile material in designated area;  Store chemicals, petroleums, gasoline, fuel and other containered items in a locked trailer or shed;  Sweep streets immediately to remove any sediment tracked from trucks exiting the site;  Cover soil stockpiles if windy conditions occur;  Watering from watering trucks will immediately infiltrate into soils;  Utilize a water truck to dampen exposed soils and prevent windblown nuisance dust;  Maintain and clean sanitary facility for worker use;  Nuclear gauge will be performed by a certified nuclear gauge operator;  Nuclear gauge will never be left where there is a possibility of getting struck by equipment;  Nuclear gauge is regulated under US Nuclear Regulatory Commission guidelines and rules;  Roadway Contractor shall perform biweekly inspections;  Maintain spill kit on site.  Waste concrete from concrete trucks will always be washed into a poly lined containment area where it will be properly removed from the site and disposed of off-site at a concrete waste facility.  Waste asphalt pavement from asphalt trucks will always be dumped into a polylined containment area where it will be properly removed from the site and disposed of off- site at an asphalt waste facility.  Nuclear gauge is regulated under US Nuclear Regulatory Commission guidelines and rules; BMP SPECIFICATION SOURCE/REFERENCE SHEET Source: General Permit for Stormwater Discharges Associated with Construction Activity Permit Number MTR100000 Montana Department of Environmental Quality, effective January 1, 2018.
