Miller INTRODUCTION We are pleased to submit this Task Order for the above referenced project. Our proposal and cost estimate are based on the expanded scope of work for soil removal under the sidewalk near the subject Heebs East Main Grocery property and DEQ's request for revisions to several documents. The additional documents include the following: • Amended Voluntary Clean-up Plan, • Construction Completion Report, and, • Amendments to the Operations and Management Plan. SCOPE OF WORK This Modification is for the following work, which is outside the scope of our previously approved Task Order. Tetra Tech will conduct the following work tasks to close out the Heebs East Sidewalk contaminated soil removal project. • Prepare an Amendment to the 2013 CMC Voluntary Clean-up Plan (VCP) to address the specifics of the Heebs East Sidewalk soil removal project. We anticipate that this will be a several page long document that identifies the location of the excavation area and the unique aspects of the soil removal project. • Prepare a Construction Completion Report that describes the excavation work that was conducted and the volume of soil and concrete that was removed and their ultimate disposal method. The estimated volume of soil that would have needed to be excavated to remove all contaminated soil within the footprint of the excavation will also be estimated. A summary of all soil samples collected at the site will be provided. • Update the Operation and Maintenance Plan for the CMC Bozeman site to address DEQ comments. • The soil excavation project required excavation and disposal of more soil than originally anticipated. Our original scope of work called for removal of the sidewalk and the top six inches of soil. Contaminated soil was observed at depths up to 4 ft. below ground surface, resulting in a total of 94.5 cubic yards of soil plus the sidewalk being excavated and transported to the Valley View landfill for disposal. This resulted in four moblizations Task Order—September 2014 Additional Tasks CIVIC Bozeman—Heebs East Sidewalk removal September 30, 2014 by Tetra Tech and Schroeder Contracting due to asbestos being reported in lab samples, despite no visual evidence being recorded during field screening. For costing purposes, we have assumed the following: • The VCP Amendment will be no more than three pages long. • The Construction Completion Report (CCR) will be no more than 15 pages long (not including appendices) and will in large part be pulled from our 2009 CCR. • Amendment of the O&M Plan will require revision or replacement of several paragraphs. • DEQ will accept the level of effort outlined above for these documents and only one round of revisions will be necessary to respond to DEQ comments. Reporting Tetra Tech will submit the following documents for City review prior to DEQ submittal. • VCP Amendment • Construction Completion Report • Amended O&M Plan SCHEDULE Upon written acceptance of this proposal we propose to initiate work on all three documents simultaneously. These draft documents will be submitted to the City within three weeks of receipt of authorization to proceed. COST ESTIMATE Tetra Tech estimates that our costs to perform the scope of work outlined above on a time and materials basis is $45,868. This includes $39,758 of additional soil excavation and disposal costs. We appreciate the opportunity to provide this proposal and look forward to closing out this project. Special Conditions Execution of this Task Order constitutes acceptance of all terms and conditions contained in Tetra Tech's Professional Services Agreement with the City of Bozeman, dated May 29t`', 2014.
