Bozeman Bozeman,Montana 59715 Subject: FTA Grant Application, Parking Garage Dear James, In preparation for the application for FTA grant funding for the described project,I have started coordination with FTA in Denver, as well as researched the FTA web site for application requirements. As you may be aware,the application process has numerous components. In our meeting scheduled for October 26th,we will attempt to address several application milestones. In my discussions with our FTA contacts,the application process will require multiple transmittals starting with the initial application items addressed below. Following are items we will need input and/or that require City action: fTh 1. City must complete Request for Addition to Vender File(See attached) 0 2. City complete com lete the Appendix A UMTA Civil Rights Assurance form(see attached). --1 3. City complete the Appendix B DOT Title VI Assurance form(see attached). D J 4. City Complete Transportation Electronic Award&Management Process(TEAM Designation of Signature Authority form and TEAM User Access Authorization form (See attached) 5. City Attorney review and process the Authorizing Resolution (Example attached) Regarding Items 1 through 4,please execute the documents and return to this office at your convenience. We will take care of forwarding it to FTA in Denver. Item No. 5 is a document that Paul Luwe needs to review and, as he feels appropriate,plan on processing through the City Commission. Thefollowing information is necessary to assist us to complete the Civil Rights compliance paperwork: 6. A list of any active lawsuits or complaints naming the applicant,which allege discrimination on the basis of race, color, or national origin with respect to service or other transit benefits. The list should include: the date the lawsuit or complaint was "Providing resources in partnership with clients to achieve their goals" i MORRISON MAIERLE,INC. filed; a summary of the allegation;and the status of the lawsuit or complaint,including whether the parties to the lawsuit have entered into a consent decree. 7. A description of all pending applications for financial assistance, and all financial assistance currently provided by other Federal agencies. 8. A summary of all civil rights compliance review activities conducted in the last 3 years. The summary should include: the purpose or reason for the review; the name of the agency or organization that performed the review; a summary of the findings and recommendations of the review; and, a report on the status and/or disposition of such findings and recommendations. Items 1 through 4 are necessary for us to start the application process. There is quite a bit of additional information that will need to be generated and as necessary,processed through the City for action or approval. We have been informed by FTA that the application process is interactive and that submittal of a single"packet"of documents is not standard procedure_ FTA asked that we pre-screen as much of the application documentation as possible to ensure compliance with their application standards. For your information, I have attached the following outline of the grant application process requirements. Labor Union Description(s) (including information about previous DOL certifications that may apply to this project) 9.Environmental Review a. Date of FTA's signing of FONSI, or b. Date of FTA's signing of ROD,or c. Grant applicant's CE recommendation if neither a nor b above applies 10. STIP-Date of most recent approval by FTA(including any amendments) (please provide projects'page numbers) 11.Request for copy of Master Agreement, if grant applicant does not have latest. 12 Information confirming that urbanized area formula funds are programmed(i.e., are in an approved STIP) r.! : MORRISON Asa:':.!MAIERLE,INC. B. Approval Prereauisites (on record with FTA, or to be submitted with application and updated as appropriate) 1. Opinion of Counsel (for first time grant applicants) 2.Authorizing Resolution (for first time grant applicants) 3. Information supporting financial capacity, technical capacity, satisfactory continuing control, maintenance capability) (for first time applicants) 4. Any outstanding oversight findings resolved or resolution plan and schedule set 5. Current annual Certification and Assurances. Grant activities in the application are covered by the Certifications and Assurances(that is, for example,if project includes acquiring real estate, then certification on real property has been made). 6.
