Revisions to
Standard Form of Agreement Between Owner and Architect with
Standard Form of Architect's Services
This Amendment No. 6 revises the AIA Document B 141 contract between StudioFORMA Architects PC
and the Bozeman Public Library, which was originally dated.tune 21, 2002, with Overland Partners Inc.,
and was transferred to StudioFORMA Architects PC on December 15, 2004. The Attachment listed
below shall be revised as follows:
ARTICLE 1.1.2 PROJECT PARAMETERS
Revise to read as follows:
1.1.2.5 The financial parameters areas follows:
1 Amount of Owner's overall budget for the Project to replace the roof as described in
Attachment No. 2 to this Agreement(Article A.2.7 Hail Damage Services), including
the Architect's Compensation is $24,490.00.
.2 Amount of the Owner's Budget for the Cost of the Work to be completed in the
Construction Phase to replace the roof, excluding the Architect's compensation, is:
$0.00.
1.1.2.6 The time parameters are:
(Identify, if appropriate, milestone dates, durations or fast track scheduling.)
The Bid/Construction Documents Phase of this Amendment shall be complete prior to February 9, 2012.
The Construction Phase of this Amendment shall commence not later than July 5,2012 unless otherwise
determined later by mutual consent of the Owner. The Construction Phase shall be complete no later than
August 31, 2012.
ARTICLE 1.6 COMPENSATION
Revise to read as follows:
1.5.1 For the Architect's services for all work described under Attachment 2 the Owner shall
p!qy a lump sum compensation of$24,490 for services as shown on attached Sheet AS07-1. Reimbursable
expenses are included within this figure with the exception of printing of Bid/Construction Documents
which will be invoiced at actual costs.
ARTICLE 2.8 SCHEDULE OF SERVICES
Revise to read as follows:
2.8.1 Design and Contract Administration Services beyond the following limits shall be
provided by the Architect as a Change in Services according with Paragraph 1.3.3:
.2 a minimum of four(4) visits per week to the site by the Architect over the duration of
the Project during the construction phase. Coordination meeting with the General Contractor

shall average three 3 times per week over the course of the construction phase.
All other provisi®nrs of Article 2.8.1E shall remain. in effect. All services listed in Article 2.8.2,
if required, 5haff Poe provided by the Architect with®rnt a Change in ,Serviem and shall be
included in the Contract Administration Phase fee.
ATTACHMENT NO. 2-DESCRIPTION OF ADDITIONAL SILE11-1 DICES
Add Article A.2.7 as follows:
ARTICLE A.2.7 HAIL DA11i1AGE SERVICES
Definition: Under this Additional Service, StudioFORMA Architects PC shall provide the services
described below to complete a re-roofing of the Bozeman Public Library due to hail damage that
occurred on June 30,2010.
Tasks: The tasks performed by StudioFORMA Architects PC shall be as shown on the attached Exhibit
AS07-l.
Deliverables: The deliverables from StudioFORMA Architects PC shall be as follows:
i. Bid/Construction Documents for a"like kind"replacement of the existing metal roofing system.
2. Bid/Construction Documents for a single-ply membrane roofing system as an optional
replacement of the existing metal roofing system.
3. Coordinate with the City ofBozeman's Superintendant of Facility Services and the Montana
Municipal Interlocal Authority(MM1A)regarding coverage under the applicable MMIA
Memorandum of Property Coverage.
4. As part of Contract Administration and as part of the Deliverables,the Architect shall coordinate
with the Library Board of Trustees,the Library Director,all pertinent agencies or departments of
the Owner, and the General Contractor once selected to develop specific mitigation measures,
e.g., securely covering all Library equipment and materials,to ensure all construction activities
do not interfere with operations of the Library. In addition,the Architect shall work with the
Library Board of Trustees,the Library Director,the City of Bozeman Superintendant of Facilities
Services,and the General Contractor once selected to develop an agreed upon schedule for hours
of construction. At least three (3)times per week during the Construction Phase,the Architect
shall inform the Library Director and City of Bozeman Superintendant of Facilities Services of
the effectiveness of all mitigation measures and of any necessary changes to the agreed upon
hours of construction.
