THIS AGREEMENT is made as of this day of C �' , 2011, between THE
CITY OF BOZEMAN, a Municipal Corporation, Bozeman, Montana, her referred to as OWNER and Great
West Engineering, Inc., an Engineering Consulting F rrn of Helena, Montana, herein referred to as ENGINEER.
WHEREAS, the parties have entered into a Professional Services Agreement dated December 7, 1998
herein referred to as Original Agreement for professional engineering services, and
WHEREAS, the parties desire to further amend the provisions of this Agreement.
NOW, THEREFORE, IN CONSIDERATION OF THE MUTUAL COVENANTS CONTAINED
HEREIN, the parties agree as follows:
TASK ORDER FF will be included with Exhibit A Scope of Services for the remainder of the
project as of August 29, 2011.
2. Section 17 — Anti - Discrimination Policy for Contracts and Other Agreements and Purchases will be
added to the TERMS AND CONDITIONS to read:
17. Anti-Discrimination Policy for Contracts and Other Agreements and Purchases
The Engineer agrees not to discriminate in the fulfillment of this Agreement on the basis of
race, color, religion, creed, sex, age, marital status, national origin, or actual or perceived sexual
orientation, gender identity or disability.
The Engineer agrees this requirement shall apply to the hiring and treatment of the Engineer's
employees and to all subcontracts.
Except as specifically amended herein, the original agreement shall remain in full force and effect and
the Parties shall be bound by all terms and conditions therein.

Task Order FF
Class IV Landfill Closure
Dear Dustin:
Please find enclosed two initialed originals of Task Order FF. Task Order FF
services include design, bidding, inspection and construction management
services for the closure of the Class IV area. It also includes preparation of
the Post-Closure Plan amendment requested by the Montana DEQ in their
March 7, 2011 letter. Our initial estimate of construction cost is approximately
$150,000. If the task order is satisfactory, please initial and return one to my
office.

Scope of Services
This Exhibit outlines our Scope of Services to perform design, bidding, construction
management and inspection services for proposed Class IV Area Closure project at the
landfill.
Consultant services will include the following:
1 Preparation of Addendum to Post-Closure Plan for monitoring of alternative final
cover performance per DEQ letter dated March 7, 2011.
2) Topographic survey of Class IV area and determination of remaining capacity in
unit.
3) Prepare final design plans for closure and borrow area design.
4) Submit final design plans and technical specifications to Montana DEQ for
approval.
5) Prepare specifications, bidding and contract documents and distribute to
interested contractors and bid exchanges.
6) Conduct pre-bid meeting with interested contractors, answer questions during bid
process, and prepare addenda, if necessary.
7) Review bids for accuracy, prepare bid tabulations, and make recommendation for
award of contract. Review bonding and insurance information, prepare Notice of
Award, Contract Agreement and Notice to Proceed.
8) Conduct pre-construction conference with Contractor and City officials.
9) Review shop drawings and submittals from contractor.
10) Provide construction staking for contractor.
11) Conduct periodic site inspections during construction to determine compliance
with specifications and plans. Conduct compaction testing and observe other
construction activities. Estimate includes 10 days of field inspection during
project construction.
12) Review pay estimates and coordinate with City and Contractor during
construction.
13) Close-out project and prepare as-built drawings.
14) Prepare closure certification report and submit to DEQ for approval
Compensation
The Consultant will perform this task on an hourly rate basis not to exceed $33,000.
The scope of work does not include soil laboratory testing fees that will be necessary for
the project. Those costs will be borne directly by the City.
