CHANGE ORDER INSTRUCTIONS A. GENERAL INFORMATION This document was developed to provide a uniform format for handling contract changes that affect Contract Price or Contract Times. Changes that have been initiated by a Work Change Directive must be incorporated into a subsequent Change Order if they affect Price or Times. Changes that affect Contract Price or Contract Times should be promptly covered by a Change Order. The practice of accumulating Change Orders to reduce the administrative burden may lead to unnecessary disputes. If Milestones have been listed in the Agreement,any effect of a Change Order thereon should be addressed. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Times, a Field Order should be used. B. COMPLETING THE CHANGE ORDER FORM Engineer normally initiates the form, including a description of the changes involved and attachments based upon documents and proposals submitted by Contractor, or requests from Owner,or both. Once Engineer has completed and signed the form, all copies should be sent to Owner or Contractor for approval, depending on whether the Change Order is a true order to the Contractor or the formalization of a negotiated agreement for a previously performed change. After approval by one contracting party, all copies should be sent to the other party for approval. Engineer should make distribution of executed copies after approval by both parties. If a change only applies to price or to times,cross out the part of the tabulation that does not apply. Removing this level of the pump station will increase the effort for some maintenance activities; however, plant staff believe the cost savings outweigh the future maintenance costs. Documentation included for this change is CPR 04 showing the breakdown of the costs and recommendation. CPR 20 Glass Lined Plug Valves This change provides glass lined plug valves in lieu of standard plug valves at some locations on sludge lines. Documentation included for this change is CPR 20 showing the breakdown of the costs and recommendation. CPR 22 Reduce Aerial Photos The number of aerial photos required by the contractor has been reduced to one set of photos at the time of project completion. Documentation included for this change is CPR 22 showing the breakdown of costs. CPR 24 Storm Sewer Cover This change provides for additional cover over a portion of the storm sewer. Documentation included for this change is CPR 24 with a detailed breakdown and recommendation. CPR 25 Riprap Material and Extension This change provides for additional riprap along the East Gallatin River and modifies the type of material from concrete rubble to rock to meet permitting requirements. Documentation included for this change is CPR 25 with a detailed breakdown and recommendation. Documentation included for this change is CPR 26 with cost breakdown and recommendation. CPR 28 Electrical Modifications This change modifies the routing of supply feeders and conduits, eliminates the need for existing Switch Board No. 2, and provides a modified feed to Lift Station No. 3. Documentation included for this change is CPR 28 with drawing sheets, a breakdown of costs and a recommendation. CPR 29 Waste Gas Burner This change adds a waste gas burner into the project to allow for construction of the headworks facility. This item was removed from the contract when the contract was awarded as some of the bid items were not awarded. Documentation included for this change is CPR 29 with a detailed breakdown of the costs. CPR 30 Electrical Service Conduits This change removes the installation of the electrical service conduits from the contract. NorthWestern Energy will be installing the conduits at no cost to the city as part of the allowance provided in the new service agreement. Documentation included for this change is CPR 30 with detailed breakdown of the costs. CPR 33 Clarifier Concrete This change provides additional thickness of concrete for the secondary clarifiers slabs in a location where multiple mats of rebar intersect. Documentation included for this change is CPR 33 with a detailed breakdown of the costs and recommendation. CPR 36 Utilidor Access This change provides for a light weight access hatch for Terminal Box 2 on the Utilidor to allow for ease of egress and entry. Documentation included for this change is CPR 36 with a detailed breakdown of the costs and recommendation. CPR 42 Flow Meter Changes This change modifies the type of flow meters utilized in a portion of the project as the meters scheduled to be utilized have been found to not work on glass lined pipe at the facility. Documentation included for this change is CPR 42 with detailed breakdown of the costs and recommendation. CPR 43 Plug Valve—4' This change adds one 4" plug valve to allow for ease of maintenance and operation in the future. Documentation included for this change is CPR 43 with detailed breakdown of the costs. CPR 44 RASMAS Demolition Change The footing and wall connection to the existing RAS/WAS structure required modification as the condition of these elements was found to be different than anticipated.
