This Task Order is dated 8/18/2022 between the City of Bozeman Facilities Department and Jackson Contractor Group (Contractor). The following representatives have been designated for the work performed under this Task Order: City: Max Ziegler, Facilities Project Coordinator Contractor: Nick Van Tighem, Jackson Contractor Group SCOPE OF WORK: The scope for this task order is detailed in the attached Bozeman Swim Center Immediate Repairs Proposal Scope of Work (Appendix A) – Task Order Number #2022-01. COMPENSATION: Jackson Contractor Group will bill for its services on a time and materials basis at rates established in the Facilities Contractor Term Contract with a project total Rough Order of Magnitude estimate of $46,578.67. Jackson Contractor Group shall submit invoices to the City of Bozeman for work accomplished during each calendar month. The amount of each monthly invoice shall be determined on the “time and materials” method whereby Jackson Contractor Group will calculate the cost of labor, materials, and expenses during the invoicing period. The provisions of the Professional Services Agreement shall govern the Work. This includes adding a steel plate to the truss pin connections at all areas where the top or bottom chord is split. It also includes the repair for the two locations where the light gage plate washer is missing, and a per DCI’s report – Roof Truss Immediate Repairs – Bozeman Swim Center 1211 W Main Street Bozeman. o Price to Provide Repair - $30,000.00 Provide and Install Ceiling System repairs to meet seismic requirements as described on Page 29 of Cushing Terrell’s report from August 8, 2022 labeled ST-1 – Replace suspension system as needed to meet seismic requirements. This work is to repair or replace ceiling tile suspension wires from grid to structure that is less than four (4) wraps of wire. While there is no description of this work in the reports provided by DCI or Cushing Terrell, this method of repair was discussed with DCI as an appropriate repair. Existing wires that are less then four (4) wraps that have a tag end that can produce the remaining needed wraps will be wrapped accordingly, wires that do not have a tag end that can produce the remaining needed wraps, will have new wire installed. o Price to Provide Repair - $4,200.00 DocuSign Envelope ID: CF46F7DD-AFA8-4A59-8313-6FA2BB3B26CF PHONE: 406-577-2772 146 LAURA LOUISE LANE BOZEMAN, MT 59718 FAX: 406-587-4282 Equal Opportunity Employer Provide and Install Roof Ridge Cap Repairs as described on Page 25 of Cushing Terrell’s report from August 8, 2022 labeled ST-1 – Install a single ply membrane draped over the exposed area of ridge cap. Secure membrane in place around its perimeter using threaded fasteners and insulation plates which penetrate through all layers of roofing and engage into the main roof deck. This work was quoted and will be performed by Ace Roofing Bozeman – Ace proposal attached. o Price to Provide Repair - $4,998.85 Summary Pricing for these three (3) Immediate Repairs o Subtotal of above - $39,198.85 o Fee and Management - $5,879.82 o Building Permit estimate - $1,500.00 Can be removed if paid for by City – no plan review fees, just building permit. o ROM total for Immediate Repair Work $46, 578.67 Jackson anticipates that this work can be performed start to finish with certificate of occupancy for this work in four (4) weeks. If the procurement of the steel plates for the truss repairs is delayed due to supply chain issues or of special treatment of steel (powder coating), schedule may be adjusted. Jackson can commence this work as soon as the building permit and Notice to Proceed is issued. Please let me know if there are any questions with this proposal. Manufacturers state that the price to obtain materials will be the price in effect on the date of shipment, and they will not commit to firm delivery dates. Accordingly, if there is an increase in price of materials between the date of this proposal and date of delivery of the affected materials, the contract amount shall be increased to reflect the additional cost incurred by Ace Roofing to obtain the materials upon providing notice and documentation of the price increases.
