Approval is based on plans and specifications received under tine sea[ of you, James R. Nickelson, A. Burgett, 14255 PE; and Michael 1V'v'. an expansion of the capacity of the existing Lyman Crook Spring vV'at_;- (addition of a new spring lateral) and upgrades to the existing Lyman C.-,'­­t-1 Water Treatment Plant (WTP). The new spring lateral will consist of approximately 20 LF of 8-inch stainless steel well screen that will be piped to a new spring box through approximately 308 LF of 12-inch PVC pipe (the existing Lyman Creek Spring laterals will also enter the spring box through a separate pipe). A bypass will be installed on the new spring lateral, and a hydrant will be installed to allow for Ground Water Under the Direct Influence of Surface Water (GWUDISW)testing. The Lyman Creek (WTP)will be upgraded to increase the hydraulic capacity of the plant and improve the chemical injection treatment systems. WTP plant piping in the inlet control building will be increased from 8-inch to 12-inch pipe. The sodium hypochlorite and hydrofluosilicic acid feed equipment will also be updated as part of this project, along with various water system appurtenances and heating and ventilation improvements in the WTP. Page 1 42 Enforcement Division Pcrnjitting&Compliance Division - Planning,Prevention&Assistance Division - ReinedintionDivision Approval is also given with the understanding that any deviation from the approved plans and specifications must be submitted to the Department for reappraisal and approval. Within 90 days following completion of the project a complete set of"as-built" record drawings must be signed, stamped, and certified to be constructed in accordance with approved plans and specifications, and submitted to the Department by the project engineer. The project may not be put into use until the project engineer certifies by letter to the Department that the activated portion of the project was inspected and found to be constructed in accordance with the plans and specifications approved by the Department. Additionally, and Application for Change of Appropriation Water Right form must be submitted to the Montana Department of Natural Resources and Conservation (DNRC)for an additional point of diversion. Construction of this project must be completed within three years of this date. If more than three years elapse before completing construction, plans and specifications must be resubmitted and approved before construction begins. The applicant is responsible for compliance with all applicable federal, state, local, and tribal law, regulations, and ordinE iccc.
