Date: March 28,2021 The City of Bozeman is requesting proposals from all interested parties for above referenced project. Attached is a preliminary scope of services and Qualification Evaluation Criteria to aid you in the completion of your proposal. The proposals should include the following: 1. Project management chart. 2. Proposed general time schedule, including start and completion dates. 3. Specific current workloads of the proposed project team members. 4. At the discretion of the firm,no more than five typewritten single spaced pages,more fully describing the firm's qualifications for the specific project. 5. At the discretion of the firm, a company brochure. 6. A completed and signed Non-Discrimination and Equal Pay Affirmation Form (REQUIRED) Discrimination in the performance of any contract awarded under this invitation on the basis of race, color, religion, creed, sex, age, marital status, national origin, or actual or perceived sexual orientation,gender identity or disability is prohibited. This prohibition shall apply to the hiring and treatment of the awarded entity's employees and to all subcontracts.Every entity submitting under this request for proposals must sign and return the required affirmation. Respondents represent they are,and for the term of this Agreement will be,in compliance with the requirements of the Equal Pay Act of 1963 and Section 39-3-104, MCA (the Montana Equal Pay Act). The city will review the documents and may interview selected firms before making the final selection. CITY OF BOZEMAN NORTH 15TH AVENUE IMPROVEMENTS FROM PATRICK TO TSCHACHE - CONSTRUCTION MANAGEMENT AND INSPECTION SERVICES PRELIMINARY SCOPE OF SERVICES The City of Bozeman is seeking Proposals from firms qualified to provide professional engineering construction management and inspection services. The project includes the construction of improvements to North 15'Avenue between Patrick and Tschache Streets. The preliminary scope of services is as follows: ♦ Plans for the project were completed and approved for the underlying property owner by KLJ. The City has received approval to use those plans to construct this section of 151. The approved plans contained a number of other streets and improvements which may necessitate minor editing to sheet numbers,names, etc.,but the design is complete and will be utilized as is. ♦ The plans were previously approved by the DEQ and the City. Resubmittal for an updated approval to DEQ will be required. ♦ A wetland permit was completed and approved for the project but the wetland mitigation was never completed. Like the plans,the wetland permit covered the larger underlying project. A resubmittal for this portion of the project will need to be completed. Credits will be purchased from a mitigation bank. ♦ A complete set of Contract Documents and Specifications manual will need to be prepared for use on the project. The project will be bid electronically using QuestCDN, so those requirements will need be included in the documents. The documents also need to include a draft SWPPP that has been approved by the City stormwater division that the contractor will be required to submit for approval from the DEQ and utilize on the project. ♦ Provide compete construction management and inspection services for the project. Coordinate with the City's material testing consultant to for all necessary construction quality assurance and acceptance testing. Surveying can either be done by the selected consultant or be required of the contractor as part of the construction contract. ♦ Provide project bidding and award assistance as necessary through the QuestCDN system. ♦ Complete and provide all project closeout documentation,inspections,certifications, and record drawings after the completion of construction. The City desires to complete the proposed work in a timely manner. Proposals must clearly indicate the firm's anticipated schedule given their staffing,and current and projected work load commitments. Proposals must clearly identify any elements of the proposed scope of work that would not be provided by the respondent. Any sub-consultants to be used must be identified along with a description of the past working history between the firms. This information will be used to evaluate the possibility of multiple contracts/consultants to accomplish the complete scope of work. QUALIFICATIONS EVALUATION CRITERIA Respondents to requests for proposals shall be evaluated on the following criteria and the requirements of 18-8-201 et seq. MCA with the selection committee choosing,in its opinion, the most qualified firm to provide the services required for the proposed project. The qualifications of professional personnel to be assigned to the project. Related experience on similar projects; Capability to meet project time and budget requirements; Location of firm in relation to project; Present and projected workloads; and Recent and current work for the City of Bozeman. The selection will be based on an evaluation of the written responses to the request for proposals and interviews of selected firms if deemed necessary. Award will be made following contract negotiations to the most qualified firm at a price which the City determines to be fair and reasonable taking into account the estimated value of services to be rendered, as well as the scope, complexity and professional nature thereof. If the City is unable to negotiate a satisfactory contract with the firm initially selected at a price the agency deems to be fair and reasonable,negotiations with that firm will be formally terminated and the agency shall select another firm, in accordance with 18-8-204 MCA, and continue until an agreement is reached or the process is terminated. All proposals must be submitted complete and contain the information required as stated in the "Request for Proposals." Attachment A NONDISCRIMINATION AND EQUAL PAY AFFIRMATION (name of entity submitting) hereby affirms it will not discriminate on the basis of race, color, religion, creed, sex, age, marital status, national origin, or because of actual or perceived sexual orientation, gender identity or disability and acknowledges and understands the eventual contract will contain a provision prohibiting discrimination as described above and this prohibition on discrimination shall apply to the hiring and treatments or proposer's employees and to all subcontracts.
