Park MEETING DATE: July 13, 2020 MEETING TYPE Consent
RECOMMENDATION: Approve Change Order #2 for the Repair of the Anderson Pavilion at Bogert Park.
BACKGROUND:
On July 15th, 2019 the City Commission directed staff to proceed with repair of the Anderson Pavilion at Bogert Park (formerly known as the Bogert Pavilion) following a partial roof collapse that occurred on March 17th, 2019. Martel Construction was selected through a competitive RFQ / RFP process and awarded a Construction Contract for General Contractor / Construction
Manager services on November 4th, 2019. Minor changes to concrete work related to the
relocation of the electrical service panel are necessary. Change Order #2 reduces the need for saw cutting and replacement concrete related to relocating the electrical service to the west side of the fire service wall.
There are no changes to the project schedule. UNRESOLVED ISSUES: None.
FISCAL EFFECTS: This adjustment will result in a reduction of ($659) in the overall cost of the project. ALTERNATIVES: As suggested by the City Commission.

GENERAL INFORMATION This document was developed to provide a uniform format for handling contract changes that affect Contract Price or Contract Times. Changes that have been initiated by a Work Change Directive must be incorporated into a subsequent Change Order if they affect Price or Times.
Changes that affect Contract Price or Contract Times should be promptly covered by a Change Order. The practice of accumulating Change Orders to reduce the administrative burden may lead to unnecessary disputes.
If Milestones have been listed in the Agreement, any effect of a Change Order thereon should be addressed. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Times,
a Field Order should be used. B. COMPLETING THE CHANGE ORDER FORM
Engineer normally initiates the form, including a description of the changes involved and attachments based upon documents and proposals submitted by Contractor, or requests from Owner, or both.
Once Engineer has completed and signed the form, all copies should be sent to Owner or Contractor for approval, depending on whether the Change Order is a true order to the Contractor or the formalization of a negotiated agreement for a previously performed change. After approval by one contracting party, all copies should be sent to the other party for approval. Engineer should make distribution of executed copies after approval by both
parties. If a change only applies to price or to times, cross out the part of the tabulation that does not apply.

Dear Jon:
We propose to provide all labor, materials, and equipment required to
perform the above mentioned work. Our lump sum quotation is based
on our interpretation of the instructions and scope of work
identified by the Proposal Request(s). If we have misinterpreted this information in any
way (omissions, etc.) please advise us prior to processing this quotation into a
formal Change Order. Other wise, our interpretation shall stand as correct.
We have not been directed to proceed with the work.
At this time, we are hereby requesting 0 calendar days for a time extension
Please note this proposal is valid until 7/3/20. Approval beyond that date may result in additional costs or delays.
Our proposal is based on receipt of a signed Change Order within (25) calendar days.
