(AE2S) - Sourdough and Lyman Reservoir Inspection and Evaluation EXHIBIT B to Professional Services Agreement Dated April 24, 2017 TASK ORDER NUMBER 9 Issued under the authority of Professional Services Agreement between the City of Bozeman and Advanced Engineering and Environmental Services,Inc.for:A range of professional and technical services related to operations and maintenance of the City of Bozeman Water Treatment Plant and support facilities, including but not necessarily limited to engineering design, preparation of construction documents, project bidding, and construction administration services for various improvement projects. This Task Order is dated , 019 between City of Bozeman (City) and Advanced Engineering and Environmental Services,Inc. (Contractor). The following representatives have been designated for the work performed under this Task Order: City: Brian Heaston,PE,Project Engineer Contractor: (Advanced Engineering and Environmental Services, Inc.) Brian Viall,PE,Project Manager SCOPE OF WORK: (attach additional sheet(s)as required) This Task Order is for a feasibility study and technical report with recommendations in association with the inspection of the 4.0 MG Sourdough and the 5.3 MG Lyman reservoirs, generally referred to as: Bozeman Sourdough & Lyman Reservoir Inspection and Evaluation Contractor's Project Number:P05097-2019-007 The scope for which is generally described as follows: En2ineerinLy—Contractor's Responsibilities: Phase 020—Study and Report Phase 1. Review available record drawings 2. Select wet inspection method 3. Hire and manage inspection services and provide reporting/documentation 4. Conduct Site visits for visual inspection and documentation of exterior site and structural conditions. a. To the extent possible,determine if the Sourdough Reservoir is leaking potable water into the embankment earthwork around the tank. b. Depending on findings, provide a monitoring well installation plan and testing recommendations to definitively determine if groundwater is influenced by reservoir leak. 5. Review, evaluate and summarize findings of site and tank inspections 6. Based on findings from evaluation,prepare cost estimates to replace and/or rehabilitate each facility. 7. Prepare Technical Memorandum containing inspection results, reservoir drawings, evaluation results, conclusions and recommendations 8. Present findings to City of Bozeman staff City's Responsibilities: 1. Provide access at each reservoir site. 2. Modify distribution operations during inspections as deemed necessary by City staff. 3. Provide Sanitary Survey data on the reservoirs for last three inspections. Documentation 1. Technical memorandum of findings, including the following: a. Summary of site and reservoir inspections b. Compiled record drawings c. Engineer's Opinion of Probable Construction Cost (EOPCC) for rehabilitation and replacement alternatives d. Conclusions and Recommendations COMPENSATION: The total cost for the engineering, documentation, and materials associated with this Task Order is $46,872.00. Contractor shall be reimbursed on a Time&Materials basis,as detailed in Attachment A—Hour and Fee Estimate by Phase and Task. Contractor shall invoice no more often than monthly for services provided in the prior month. The provisions of the Professional Services Master Task Order Agreement and any Special Terms and Conditions and/or Exhibits or Attachments to this Task Order shall govern the Work.
