Employee Assistance Program Agreement f . .Ii , ; J Agreement for city of Bozeman Employee Assistance Proqram This agreement made this 1st day of October. 1993 between the city of Bozeman and Community Counseling Service, Inc. of Bozeman, Montana. Statement of Services: Community Counseling Service, Inc. shall provide counseling and referral services for all full-time city employees and immediate family members. The services shall encompass counseling, with respect to substance abuse, emotional distress, and family/marital problems, and the matching of the employee or family members with the referral center which is the most appropriate treatment source and which most favorably relates to the insurance coverage held by the employee and/or his ability to pay. The covered employees and family members may utilize the service at Community Counseling Service, Inc. office or by telephone. Communi ty Counseling Service, Inc. shall provide appropriate office space, printed materials, telephone service and office equipment necessary for the performance of the services. Community Counseling Service, Inc. will also train all levels of the City of Bozeman management on the premises in the awareness and recognition of the problem areas listed above and in the most effective utilization of the Community Counseling Service, Inc. services. Community Counseling Service, Inc. shall provide the required education of and information to the employee population at large, as well as all levels of management with respect to the services provided and the utilization thereof. Term of Service: Regular services under this agreement shall commence on October 1, 1993 and continue until September 30, 1994. Police and Fire Department Representative: All services rendered hereunder are subject to coordination with Larry Conner (Police Department) or successor as stated by addendum. Members of the Teamsters and Labor Union as well as the Superintendents and Assistant Superintendents of the Water. Sewer. Street. Garbage. Parks. Cemetery. Waste Water Treatment Plant and Water Treatment Plant: All services rendered hereunder are subject to coordination with Mike Certalic or successor as stated by addendum. ! . r " ) Members of the EmDlovees Bargaining Group. the Mid-Management Baraainina Group. and all other full-time emDloyees not included above: All services rendered hereunder are subject to coordination with Anita Johnson or successor as stated by addendum. Compensation: Charges for services mentioned herein shall be as follows: Costs for Employee Assistance Program staff travel are included in the charges for service. Pavment: The payment for services by the city of Bozeman to the Employee Assistance Program will be made for services rendered on a monthly basis. The schedule of payments for the term of this agreement shall be as forth in Addendum "A" attached hereto, and by this reference made a part hereof. Use of Information: All information obtained by Community counseling Service, Inc. staff members as a result of services provided shall be kept strictly confidential and shall remain the property of the individual. Any use of said information by.any party is prohibited unless expressed written consent for use thereof is obtained by all parties concerned. General information regarding trends and/or other problem areas will be reported as soon as possible to management and/or representatives. All statistics and material related to the City of Bozeman program shall also be kept confidential by Community Counseling Service, Inc. and the use of such material for pUblication or report shall be subject to the coordinators' prior review and written approval. Communi ty Counseling Service, Inc. reserves the right to include the city of Bozeman program statistics in the total compilation of all programs carried out by the Employee Assistance Program for reporting purposes. Extension of Agreement: The terms of this agreement may extend for a one-year period by concurrence by both of the parties involved. Failure to address the issue not less than thirty (30) days prior to the end of the agreement will result in an automatic one-year extension under the existing terms. Termination: This agreement may be cancelled by either party on written l ~ . > . , ., notification sent by registered or certified mail at least thirty (30) days prior to the effective date thereof. Payment for services performed to and including the termination date shall constitute full satisfaction of all your claims for payment.
