CHANGE ORDER
INSTRUCTIONS
A. GENERAL INFORMATION
This document was developed to provide a uniform format for handling contract changes that affect Contract
Price or Contract Times. Changes that have been initiated by a Work Change Directive must be incorporated into
a subsequent Change Order if they affect Price or Times.
Changes that affect Contract Price or Contract Times should be promptly covered by a Change Order. The
practice of accumulating Change Orders to reduce the administrative burden may lead to unnecessary disputes.
If Milestones have been listed in the Agreement, any effect of a Change Order thereon should be addressed.
For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Times,
a Field Order should be used.
B. COMPLETING THE CHANGE ORDER FORM
Engineer normally initiates the form, including a description of the changes involved and attachments based upon
documents and proposals submitted by Contractor, or requests from Owner,or both.
Once Engineer has completed and signed the form, all copies should be sent to Owner or Contractor for approval,
depending on whether the Change Order is a true order to the Contractor or the formalization of a negotiated
agreement for a previously performed change. After approval by one contracting party, all copies should be sent
to the other party for approval. Engineer should make distribution of executed copies after approval by both
parties.
If a change only applies to price or to times,cross out the part of the tabulation that does not apply.

Change Order/ Price Increase

Dear Chris,
In reference to the requested changed/added work items per conversations between
RPA and Montana Civil Contractors, we are pleased to provide the City of Bozeman with the
prices and items below.
• Item One: Tank Panel Upright
o Tank site panel (Detail 3 Sheet E-1) changed to (2)4"galvanized rigid conduit
upright supports with galvanized channel attached horizontally for panel mounting.
Posts poured in bagged concrete mixed on site. Pedestal built oversized to
accommodate future needs.
■ Price includes:procurement and installation of materials, labor and
equipment.

• Item Two: Re-Route of Fluoride Sample Line
o Additional excavation, placement, and backfill of added length of buried fluoride line
from Flowmeter vault to northwest corner of building.
o Excavation and exposure of west foundation wall. Excavation will be sized properly
to accommodate safe access for subcontractor and drill.
o Core Drill Exterior wall with 5" core drill
0 4.5" Steel casing driven through concrete screen rock (as shown in planset provided
to MCC from RPA)to interior sump pit wall. Screen rock inside casing will be
removed.
o Core drill interior wall with 4" core drill and extension into sump pit.
0 3" Schedule 40 pvc pipe to be installed in casing.
0 3" Schedule 40 pvc pipe will then be attached to sump pit wall, ran down to the floor
and along edge of wall approximately 20' and terminated.
o Exterioer and Interior spacing around steel casing and 3" schedule 40 PVC will be
grouted.
o Exterior excavation for coring will be backfilled and surface graded.

RE: 5.3 MG Water Storage Tank—Change Order#4 Summary
Bob,
Please see the Summary of Change Order#4 below.
Item#1 —Tank Electrical Panel Support: RPA agrees with this item for$1,830.15. We feel this
electrical panel support is an upgrade and matches the panel supports used at the water treatment
plant site.
Item#2—Re-route Water Sample Line: RPA agrees with this item for$17,821.00 and 3
calendar days. The location of the existing water sample line to be connected onto was shown
incorrectly on the as-builts. The contractor attempted to locate the water sample line with
excavation equipment to no avail. RPA, MCC, and water treatment plant staff worked together
to come up with an alternative plan for bringing the water sample line into the treatment
building.
Item#3 - Time Extension: 3 calendar days for C.O. work+ 8 weather calendar days due to rain
= 11 extra calendar days.
