GENERAL RTC; TION This document was developed to provide a uniform format for handling contract changes that affect Contract Price or Contract Times. Changes that have been initiated by a Work Change Directive must be incorporated into a subsequent Change Order if they affect Price or Times. Changes that affect Contract Price or Contract Times should be promptly covered by a Change Order. The practice of accumulating Change Orders to reduce the administrative burden may lead to unnecessary disputes. If Milestones have been listed in the Agreement,any effect of a Change Order thereon should be addressed. For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Times, a Field Order should be used. R. C O LETWC THEE CILANGE ORDER FORM Engineer normally initiates the form, including a description of the changes involved and attachments based upon documents and proposals submitted by Contractor,or requests from Owner,or both. Once Engineer has completed and signed the form,all copies should be sent to Owner or Contractor for approval, depending on whether the Change Order is a true order to the Contractor or the formalization of a negotiated agreement for a previously performed change. After approval by one contracting party,all copies should be sent to the other party for approval. Engineer should make distribution of executed copies after approval by both parties. Installation of BWTP supplied 20' gate inside newly installed gate panels. ■ Price includes: subcontractor mobilization, procurement and installation of materials not supplied, labor and equipment. ■ Price doesn't include:20'gate panels for either locations. MCC had to procure an additional section of pipe to be used for the added LF to the inlet and outlet pipe, above and beyond the already procured pipe that had been ordered and delivered for the project. Due to limited supply and scheduling, additional pipe was priced at a premium, but allowed the project not to be delayed any further due to manufacturing and shipping. Pipe was also sourced within reasonable distance saving in freight. o Procurement, mobilization of pipe, modification and installation of additional LF of 42" Ductile Iron pipe. o Bedding material placement. Price includes:All components incorporated with additional 5.5 LF on Inlet pipe and 6.5 LF on Outlet pipe, couplers, associated parts, materials, bedding, equipment and labor. Each price and time request is subject change should additional services be required or additional work be needed. Additionally, due to adverse weather conditions on the following stated dates, 6/2/17, 6/5/17, 6/28/17, 6/2/18, 6/18/18, 6/19/18, 6/29/18, 8/27/18, 10/4/18, 10/05/18, 10/08/18, 10/15/18 and 10/29/18.
