2) Expose Exterior Manhole Painting is an increase of$866.25.
3) Revised Flowmeter Vault is an increase of$54,454.55.
4) Additional Pipe 13 LF is an increase of$11,755.50.
5) Time extension: 9 calendar days for change order related work and 13 weather calendar days=22 total
Reason for Change Order: Changes during construction to the contract.

A. GENERAL RTC; TION
This document was developed to provide a uniform format for handling contract changes that affect Contract
Price or Contract Times. Changes that have been initiated by a Work Change Directive must be incorporated into
a subsequent Change Order if they affect Price or Times.
Changes that affect Contract Price or Contract Times should be promptly covered by a Change Order. The
practice of accumulating Change Orders to reduce the administrative burden may lead to unnecessary disputes.
If Milestones have been listed in the Agreement,any effect of a Change Order thereon should be addressed.
For supplemental instructions and minor changes not involving a change in the Contract Price or Contract Times,
a Field Order should be used.

Engineer normally initiates the form, including a description of the changes involved and attachments based upon
documents and proposals submitted by Contractor,or requests from Owner,or both.
Once Engineer has completed and signed the form,all copies should be sent to Owner or Contractor for approval,
depending on whether the Change Order is a true order to the Contractor or the formalization of a negotiated
agreement for a previously performed change. After approval by one contracting party,all copies should be sent
to the other party for approval. Engineer should make distribution of executed copies after approval by both
parties.
If a change only applies to price or to times,cross out the part of the tabulation that does not apply.

Dear Chris,
In reference to the requested changed/added work items per conversations between
RPA and Montana Civil Contractors, we are pleased to provide the City of Bozeman with the
prices and items below.
• Item One: Existing Gate Relocation &Additional Gate Install
o Removal of existing 20' gate at NE corner of property and reinstallation to SW corner
of property.
o Installation of new gate posts on N side of new perimeter fence per BWTP request.
Installation of BWTP supplied 20' gate inside newly installed gate panels.
■ Price includes: subcontractor mobilization, procurement and installation of
materials not supplied, labor and equipment.
■ Price doesn't include:20'gate panels for either locations.

* Item Four: Additional Pipe 13 LE
* Due to 30" DI tie-in pipe being approximately 7 ft. further away from planned drawings
showed, additional LF of pipe was needed. MCC had to procure an additional section of pipe to
be used for the added LF to the inlet and outlet pipe, above and beyond the already procured
pipe that had been ordered and delivered for the project. Due to limited supply and scheduling,
additional pipe was priced at a premium, but allowed the project not to be delayed any further
due to manufacturing and shipping. Pipe was also sourced within reasonable distance saving in
freight.
o Procurement, mobilization of pipe, modification and installation of additional LF of
42" Ductile Iron pipe.
o Bedding material placement.
Price includes:All components incorporated with additional 5.5 LF on
Inlet pipe and 6.5 LF on Outlet pipe, couplers, associated parts, materials,
bedding, equipment and labor.

The aforementioned prices and time allotments are based off of the described work by RPA to
MCC. Each price and time request is subject change should additional services be required or
additional work be needed.
Additionally, due to adverse weather conditions on the following stated dates, 6/2/17, 6/5/17,
6/28/17, 6/2/18, 6/18/18, 6/19/18, 6/29/18, 8/27/18, 10/4/18, 10/05/18, 10/08/18, 10/15/18 and
10/29/18. MCC respectfully requests an additional 13 weather days be granted to the project
schedule.
