This detached one-story single-family residence has a rectangular plan with a flat-roofed balcony extending over the front entrance. The one-bay facade is symmetrical and consists of a central glass-paned front entrance flanked by fixed windows with one pane. The frame construction is finished in clapboard and rests on a concrete foundation. The gable roof is covered with green asphalt shingles and features boxed eaves . Outbuildings include a garage. Heetderks , probably for rental or later sale , the significance of this house has been greatly reduced by the altered fenestration and artificial siding . It was probably originally identical to 616 S . 5th Ave . , and similar to 615 S . 5th Ave . While no structures existed in the 600 or 700 block in 1935 , according to the city directory of that year , Henry J . Vidack , a " laborer" , lived here in 1940 . Dr . Heetderks , and his wife Kathrine B . (who signed the sewer and water permits ) , lived in a more posh section of town at 426 W . Cleveland .
