SUBJECT: Change Order #1 for the Alfred M. Stiff Professional Building Remodel MEETING DATE: June 13, 2016

RECOMMENDATION: Authorize City Manager’s signature on Change Order #1 for the Alfred
Stiff Professional Building Phase 1 remodel.
BACKGROUND: The City’s FY16 approved budget contains $200,000 for an interior alteration
project within the Stiff Professional Building. This project, as approved at the March 14, 2016 City Commission meeting remodels existing underutilized space within the building to accommodate
additional staff in GIS, Engineering, and Planning and will improve staff and public interaction
efficiencies for those essential services. This project is designed to be the first phase in a larger
remodel project that will ultimately co-locate departments on the same floors to improve customer
service and improve employee efficiency and effectiveness
During the plan review process, the Building Division asked for several code compliance related
changes that increased the scope of the project. Additionally, the City asked for changes to improve the
usability and operations of the remodeled spaces including items that, if completed during this phase,
would reduce future costs to perform the same work in a later phase. Anticipated project completion will occur the last week of May 2016. The items on the change order were approved to keep the
project on time and to minimize impact to the general public along with staff that use this facility.
All the changes were reviewed and approved by the Facilities Superintendent. The City carries an
owner-managed contingency of $15,400 (10% of the approved construction amount) for this project.
FISCAL EFFECTS: The total of the requested change orders is $13,827.60 and is within the
approved owner-managed contingency amount approved in the project budget and will leave a balance
of $1,572.40. No other changes are anticipated.

Description:Proposal Request-1 Permit drawing changes
We are pleased to offer the following specifications and pricing to make the following changes:
Please see attached pricing for the changes that were made to the plans as a result of the permit drawing review for the Alfred M.
Stiff Building. I have attached the Proposal request to this COR for reference.
This price also includes extra money for fire taping the ceiling between the lower and main level which was never brought up in the
bidding process, but was required by the building inspector, also there was
a request to insulate the wall that seperates the new conference room, and the server room, which also was not in the plans. There
is also a price from the electrician for extra work to modify conduit after it was
brought to our attention by the inspector that the ceiling height was to low. Everything was built per plan to that point. An additional
door the is to be installed on the main level to keep continuity through out the
building. Pricing also includes additional sprinkler heads that need to be installed in the "Old Platt Room" on the main level per Jack
Coburn. Lastly the additional dedicated circuits for the unit heaters in the upper
level are aslo included in this price. Please review and contact Chase Dyksterhouse if you have any questions or concerns.

1. Lower Level Scope Changes: Remove one door and infill opening. Provide water service to
refrigerator location. Refer to Sheets D1.0, A1.0, A4.0, and MP2.0.
2. Main Level Scope Changes: Provide and install two safety gates in stairwells. Refer to Sheet A1.1
and Detail 11/A4.0. (Note: Provide a proposal to provide and install two gates as shown in Detail
11/A4.0. Provide a second proposal to provide and install two gates as shown in Detail 12/A4.1,
complete with hold-open hardware and electrical work required to connect safety gates to fire alarm.)
3. Low Voltage Cabling: Eliminate Cat 5e cabling. All horizontal data and voice cabling to be Cat 6
with patch panel terminations to work with Owner’s existing VoIP system. Refer to revised Page
271500-4.

with the proposed change, such as materials, labor, overhead, and profit. This proposal shall be submitted to the
requestor within fourteen (14) calendar days of the date given below.

ALFRED M. STIFF PROFESSIONAL BUILDING INTERIOR ALTERATION
2.3 COPPER WIRE AND CABLES
A. Horizontal Data and Voice Cabling - All building data and voice cable shall consist of
listed, category 6, non- plenum, unshielded twisted pair (UTP), 4-pair, 24 AWG solid copper wires. The insulation and jacketing shall be type CM. Outer jacket shall be
blue. Under-carpet wiring and flat wiring are not allowed.
B. Horizontal Voice Cabling - All building voice cable shall consist of listed, category 5e or 5e, non-plenum, unshielded twisted pair (UTP), 4-pair, 24 AWG solid copper wires.
The insulation and jacketing shall be type CM. Outer jacket shall be white. Under-
carpet wiring and flat wiring are not allowed. C. Multi-Pair Voice Cable – Multi-pair telephone cable shall be 22 AWG solid annealed
copper conductors with low density polyethylene jacket, non-plenum rated. 2.4 EQUIPMENT, TERMINATIONS, AND CONNECTING HARDWARE A. All horizontal copper data and voice cable shall terminate at patch panels. Patch
panels shall be rated Category 6 and shall be equipped with rear cable manager, icon/label holders, designation labels, cable ties and mounting hardware. Patch
panels shall be equal to Siemon HD6 series. Patch panel quantities shall be per the plans. B. All horizontal copper voice cable shall terminate onto modular type 66 blocks. Blocks
shall be rated Category 5e, shall be constructed of flame-retardent thermoplastic, shall include fanning strips with labeling surface, and shall be mounted onto a stand-off to
allow space for routing cables behind each block. Blocks shall be equal to Siemon
S66M1series. S66 block quantities shall be per the plans.
C. Equipment racks shall be 84” x 19”, aluminum, with quantity and accessories per the details on the plan. Equipment racks shall be equal to Siemon RS.
D. Multi-pair voice cables shall be provided per the plans and shall be terminated onto 66 blocks in telecommuniucations closets where identified on the plans. Cables shall be
extended into Owner’s PBX room for termination by Owner’s representative. Provide a minimum of 20’ spare cable in PBX Room.
2.5 OUTLETS
A. RJ45 Couplers, for voice and data, shall be straightangled, Siemon CT-6 or equal. Each coupler shall include a colored icon tab, white with telephone image for voice,
blue with computer image for data.
B. Couplers shall be arranged in combinations of single, dual and blanks depending upon the number required at a particular location per the contract drawings. C. Faceplates shall be single-gang, plastic, with labels and label covers. Siemon CT
series or equal. D. Colors shall be white.
